I think I'm getting this down to a science. Seal the neck with a strip of 3/4 male, 1/4 female velcro and a little tape on the back to keep the rear of the bag snug. Corner cut seems to be the best...I tried a cut further inward but my head kept on plugging it up. When I secured the bag at the base I would always try and angle it so my chin was pointed down which would cause the bag to be tilted forward. If you cut a slit on the corner then the bag could tilt forward to the point where you're looking forward (not up) and still looking directly through the slit, and that's no good. The tape on the back helps maintain ideal position.
Once I'm all sealed up I can start to insert the warm water. I can fill it up a lot because if I start to get water in my nose I can just tilt my head upward and expose my mouth and nose. I do have to use my left hand to keep the breathing hole open because the bags I use are not very sturdy. I wonder where I can get bags like in the pics Frank posted? It's a bit disappointing having to use my left hand but I was able to orgasm very quickly. In the process I would take on some water through my nose but I really liked it. It's not the same as waterboarding but it's enough to keep things safe while still getting enough dunking to fantasize. I have a cold right now too so the warm water gave me temporary sinus relief. I highly recommend it.
