I think you'll be hard pressed to find anything affordable not from China.

Outside of those, I am only aware of the german manufacturer MyStim, who as far as I can tell, only ships domestically and are at the upper end of the price range. Come to think of it, RIMBA is another brand I've seen  mentioned a lot, but they are also not cheap. I think they are based in the UK.

I bought a cheap TENS unit for ~EUR 25 a while back as a starter kit which served me well.
I have no idea how it compares to a MyStim or the ubiquitous DG Coyote (China!) and I don't feel compelled yet to switch up from it. The possibility to use an app to control the DG Coyote and create "playlists" of varying patterns, intensities and duration sounds tempting though.

   

Since you can get adapter cables for most connection types used, they are quite universal.

I would it describe it as a very high frequency vibration with an additional, piercing sting (which can be somewhat mitigated by using lube around the contact point). At low intensity, it is just a tingle, but as you get to your personal threshold level, it can make you want to buckle over while exhaling sharpy through your teeth. But in a good way, if that makes sense 😋

There are modes on my TENS unit that administer slower, pulsating shocks as well.
Imagine your cock being whipped mercilessly by a teeny-tiny cane. Not exactly my thing, but if that's something you think you'd enjoy, well, there you go.

My TENS unit has two channels, so I can use one line on two cock rings and the other to attach a pair of pads on the inside of my thighs or on both of my asscheeks. By using different "programs" on each channel, you can get some quite interesting effects, creating "waves" flowing up and down.

If you go through with it, make sure you familiarize yourself with the safety instructions. 
For instance, putting an electrode on your chest and one on the back could lead to get your heart zapped, which, as you can imagine, is not a smart idea. Nipple to nipple should be fine, as the current travels the shortest route, i.e. across or slightly below the skin, but I have not dared to try it, since I have a - very mild and otherwise harmless - heart condition. Scratch that, don't use unipolar clamps or contacts (that channel the electric flow from one contact point to the other) on the nipples, as this poses a health risk. Bipolar contacts (for instance a clamp where the current travels between the prongs) are way safer for this purpose, since the flow of current is limited to the small distance between the prongs.

Extended exposure at higher intensities also can supposedly burn the skin, but I don't think the TENS units go to a level where this becomes an actual risk. Can't speak for the dedicated e-stim models, I would assume they have a slightly wider range.

I have an electrastim. That is not cheap though.
But it is very nice. You can also connect it to external sound sources and it comes with a CD that contains some special 'music' for it.
Using programs like Audacity you could make your own.
You can get a number of peripherals for it. I have some bipolar nipple clamps, which is, as far as I understand, the only safe way to use a tense on your nipples.
The machine has 2 outputs and 9 programs. And a lot of intensity levels.
Used on the penis I can let it give me hands-free climaxes. Once, tied down, it gave me four during a one hour session. The first took a while. It was a ramping setting that made the intensity increase over a period of almost 100 sec and after the first climax, it took a few extra cycles and the next one came. And again, and again. Finally my ice lock released.

I usually have a metal loop near the tip and then either a conducting rubber loop around the basis of the scrotum, or a metal plug in the anus. Both work well.

Usually yes.
If you put the two on the same channel the shortest path for the electricity is still through the nipples, even though there is an alternative path. In terms of potentials, that would be a higher order effect and hence much weaker.
