I had on just like that, unfortunately it broke as it was quite cheep, but it held up for 4-5 years witch isn't bad considering its price.
I used a blow-up gag with it often, the tube for the gag just fit trough the mouth hole and of course held it in place very well. Just had to remove the hand pump from the gag squeezes the tube trough the ring in the front (it was a very tight fit and completely blocked the air hole) an reattach the pump. Never had any trouble breathing as the air could move trough the neck hole (even when the collar was strapped tight) and the seams also leaked some air in to the mask. It felt a little stuffy and sweaty at first but after 5-10 min I wouldn't notice it any more.
