Usually "good gags" keep your mouth open (there is a usual hood design flaw - http://www.likera.com/blog/wp/archives/10695 ) , and extend outside of you mouth, what may block the mouth hole.
I had on just like that, unfortunately it broke as it was quite cheep, but it held up for 4-5 years witch isn't bad considering its price.
I used a blow-up gag with it often, the tube for the gag just fit trough the mouth hole and of course held it in place very well. Just had to remove the hand pump from the gag squeezes the tube trough the ring in the front (it was a very tight fit and completely blocked the air hole) an reattach the pump. Never had any trouble breathing as the air could move trough the neck hole (even when the collar was strapped tight) and the seams also leaked some air in to the mask. It felt a little stuffy and sweaty at first but after 5-10 min I wouldn't notice it any more.

Size L is pretty large, men with stronger shoulders and arms than me. Perhaps I can do something with the bag and the backpiece, use different straps etc. As it is, it is harder to get in then out. If properly pulled up on the arms, it might be more secure, but with the original straps, that is not possible.
The material is kind of weak, but nice shiny pvc-ish outside and soft inside, comfortable.
Anyway, good stuff for the price, to play around with.

Interesting
anatomic latex gag Does not come cheap.
It's not firm (made from 0.4mm latex), but it should make speech almost impossible.

Some may recall this
post I made in the 'Strange Weird or Totally inappropriate gags' thread.
As a reminder this was the concept gag
- all very interesting and fantasy only
well.... someone has actually made this for real and you can buy it on eBay!
- its marketed as a
'deep throat Pony gag' and looks brilliant
the only downside is cost - $300 !!!!
Enjoy
