Can you take the mask off and/or the gag out without using hands?

Yes, even with the strap, you still can spit it out. The back strap stretches a bit, but once it’s buckled, your tounge will be able to push the 2 inches gag out beyond your teeth.
If that gag part had a bigger ball or an inflatable ball, I don’t think you could push it out.

With nose tubes or even with a head strap or harness, I think it could be inescapable. But not in its present form.

Maybe if was made into a hood, then it would be inescapable.

Or, a full face with two straps.
I did a quick chair tie while wearing the large one. Hands free, just in case. Had an enjoyable hour with no affects.
