To fix the dildo you could use a ring that fits over the tip but not all the way to the bottom, bit of strapping and a few buckles to lock it down tight. Maybe use a plumbing end cap as a cup for its base.

With the spreader bar for the feet it is possible to attach the bracelets in the same place. And to increase the pleasure ..... it is also possible to attach the bar to the chastity cage. Everything with padlocks of course
