How about a head harness similar to this, but with an attached collar:
The collar would act as a foundation, to keep any of the other straps from being pulled up. The strap over the top of the head, could keep anything from being pulled down. Perhaps, add a couple straps running from the chin strap, back to the collar, to keep it from sliding off. Of course, it would take a half-dozen padlocks to secure all of those buckles.
The materials certainly wouldn't cost much. It should be fairly simple for someone DIY one of these. I'd do it, if I still had access to the leather, dyes, and findings. I have all of the tools ... somewhere.
Still, the ultimate would be a locking hood:
Unfortunately, this is a low-rez image. But, in addition to locking buckles, it appears to be a small D-ring at the end of the mouth zipper, so a small padlock can be used to lock the zipper closed. Stuff your gag of choice in the mouth---ball, rag, pair of panties---close the zipper, and lock it!

Thanks. I like the design, and the fit sounds too delicious. If I purchased one though, it would be to disassemble it, and use the pieces as a pattern to make one in genuine leather. I'm too much of a stickler for the real thing. The eyelets would be a problem though, as I don't have a tool for ones that big. Perhaps rings of 8 oz latigo or cowhide might work. And yes, I do have the punch for cutting the slots needed for locking buckles (I think I also have a couple of those buckles left).
I had a full hood, once. It was real leather---but, poorly designed. I found it impossible to get in on properly, by myself. And all of my...<ahem>..."Lady Friends" over the years have been either subs, slaves, or disinterested third parties. No one wanted to believe that I might have a submissive side---not even the couple Domina trainees that I had.

I have this exact same one, with the long dildo, and I must be less of a Houdini because I can't get it off even using the technique described. The dildo is still fairly firm on mine, maybe if it gets more flexible over time it could come off.
However it is pretty long, pushing right up in the back of my throat like a deep-throat. So I second that this is definitely not for self-bondage sessions where hands are locked away. Depending on the angle and what I'm doing it's made me nervous a few times as it made breathing harder.
