I still don't understand how this works. Do you tie yourself up and struggle until the cd drawer ejects and releases the keys into your hand? The diagram appears to demonstrate this but they keys are far from dropping into the subjects hand.

On Linux you can just eject the cd by typing the "eject" command, put it after a sleep and it works as a timed release.

sleep 60; eject -T


that command waits 60 seconds and then ejects en retracts the cd tray. So I'm not sure why there is a need for a program to do that.

Yes, you tie yourself up and struggle, or stay as still as you can depending on how you feel (!) and it is up to you to position the keys so that once released, you have some way of pulling them into your hands. I use a piece of string that is trapped by the cd tray on the laptop, positioned so that the laptop camera captures any movement. Movement triggers additional time to your session, and once the time expires the keys are released and then you can pull the string towards you and unlock yourself.

Obviously, if you lose the string you are stuffed (literally if you have a nice butt plug in at the time) so I usually tie one end of the string to the chain on the cuffs so I don't need to keep hold of the string for the duration of the session.
