Two ideas would be so much fun to try with this but also add where the neck connection is, it connects to head and holds in rigid with neck brace so can't cheat

1. Have the piston slowly going back and forth while it connects to another piston in your ass doing the same time.

2. Have the brace connected to a sensor. Also have a dildo mounted on a pole. Now however far you deep throat it and at what point you go to is matched to a machine with a dildo in your ass. The dealer on the dick you suck means more in the ass. This combined with the earlier system on points with having to keep going to get to x amount of points to turn on a dildo connected to dick which then when you cum it drops onto a sensitive weight pad amd releases a key.
