This Java-applet (attached) will turn your phone into a vibrator.

Game console controllers can also be used this way. I don't know of any programs you can download to do this as I opted to write my own on the fly.

Xbox 360 controllers are easily controlled through Microsoft's XNA Framework. The wired ones are the easiest to work with since you don't need to buy an extra wireless receiver.

Wii's Wiimote is also pretty easy to control through wiimotelib. You do need to get a bluetooth dongle to be able to control it. The controller itself is also roughly vibrator shaped, for what that's worth.

I don't have experience programming the PS3 controller, but some Googleing suggests it is possible.
