Yeah, for several years my selfbondage sessions almost always involved piss-condom to mouth gag setup 😂

On the other hand, if you’re inverted, penis is higher than mouth, then your pee will flow. But if you can block the tube or blow into the tube, then you’ll have a bigger mess to deal with.

With that said, the piss gag tube must rest behind the tongue far enough to prevent the tongue from blocking the piss tube.

Also, the gag needs to be like a wiffle ball so that even if you do try to blow, the air will leak out and the piss will continue to invade your mouth.
